decorate as you want, I've used paws, bones, and I like to put something about the contents on the pouch....
I have pictures of all 4 of my dogs, with names underneath. My form says:
EMERGENCY INFO AND PROCEDURES:
The safety of my dogs is of utmost concern ot me, if I am in an accident, please follow these directions as closely as you can. Thank You !
If at all possible, leave dogs in their crates, they feel safe there. There are leads on each dog, or attached to their crates, PLEASE, make sure they are on a lead before letting them out of their crates, tell them "WAIT" as you open the crate, if they are scared, they will come out very quickly.

IN CASE OF EMERGENCY:
Please, take them to the nearest reputable vet for boarding. Their care will be guaranteed by any of the following individuals:
(list people you know who would be willing to do this, with a credit card, over the phone, no questions asked)
I personally list people by area, Kansas, Colorado, Kansas City, etc, so the person closest will be contacted if at all possible.

Their vet is :
I list my vet, her work, home and cell #'s. She is also authorized to make an emergency decision if need be.

If you have a dog on daily meds, note name of dog, breed, sex and what medication and dosage.

I also put in a copy of each dogs rabies certificate.

While I hope NONE of you ever needs this, it's sure a good idea for people traveling, especially alone, to have this in their vehicles, especially on the crates.
